Need Help Accessing Your Forms?

Follow these instructions, below for additional clarification on accessing your electronic forms.

1.) Find and open the email that contains your shared form. The sender will be hello@eriepsychiatry.com. We will share all forms to the email you provided upon registration.

2.) Each email is encrypted and contains a unique password that is needed to access the specific form that has been shared with you. Passwords are in RED.

3.) Highlight and copy (Ctrl + C) the password.

4.) Click the green button that reads ‘OPEN FORM’

5.) Paste the copied password.
